Any risk management package, therefore, will consist of a written description of the procedure to serve as the standard with clearly demarcated areas of respon sibilities of the involved personnel; if there is such an incident, a process to classify, a protocol for analysis and investigation protocol; a policy of informatics to both the patient and staff with a positive and fair blame approach; guidelines for management of care and service delivery more efficiently; and most importantly, learning from these incidents (Moss, F., Palmberg, M., and Plsek, P., 2000).
